The Personal Helpers and Mentors (PHaMS) program is for people with a severe mental illness that affects their ability to function on a day-to-day basis.
Clients have an Anglicare worker to support them to identify new opportunities and gain access to appropriate services and programs.
PHaMS focuses on strengths, what people can do, rather than want they can’t. PHaMS is recovery based and supports people lead a full and rewarding life in their community.
To be eligible, you have to
- Have a mental illness (which does not need to be formally diagnosed)
- Be 16 years or over
- Live in the Elizabeth area (Postcodes 5112 or 5113)
- Be willing to address your Mental Illness and other problems such as drug and/or alcohol dependency
- Be willing to attend the program voluntarily
- Be willing to work with a Personal Helper/Mentor on an Individual Recovery Plan to make life changes
- Are not residing in a residential mental health, psychiatric or correctional service
PHaMS team members support people in many ways, including
- Working with you to learn how to better manage everyday tasks such as housekeeping, managing your finances and how to use public transport
- Helping you get relationships with family and friends back on track
- Helping you build new relationships
- Accessing appropriate clinical and community supports
- Working with you to get involved in social and community activities
- Helping you to develop goals and strategies to achieve them
