Community Aged Care ProgramsAnglicare’s Community Aged Care Programs are designed
to help elderly people to remain in their own homes, by
arranging whatever personal or household support and
care they need.
Our community care options are tailored to meet people’s individual needs, preferences and circumstances, and will be flexible enough to change if the situation requires. Transition Care Packages offer care and support after illness or other crisis, to give the elderly person the opportunity to see how well they manage on their own, or what support they might need in the future. Anglicare offers Community Aged Care to all areas of metropolitan Adelaide. Anglicare also co-ordinates packages for frail elderly people who are at risk of homelessness. To apply for Community Aged Care Packages (CACPs) and Extended Aged Care at Home (EACH) Packages, contact the local Aged Care Assessment Team (ACAT).
